To contact Judith please e-mail me using the contact form below to see if openings are available.  Opening documents for your intake will be e-mailed only.  Please print, fill out, sign, and email the document(s) back to Judith 3 days prior to the appointment.  We cannot move forward into your appointment without this document(s) in a timely manner.  Please set up a Paypal account to pay for services in advance of contacting Judith.  A 24 hour notice of cancelation in writing is required.  You will be charged once you are scheduled with Judith.  Cancellation of less than 24 hours will result in being fully charged for Judith's time.
